What to bring

If you are a first time patient of Seattle Center for Structural Medicine, please download and complete our New Patient Form (PDF). Please bring this completed form, along with your doctor's referral to your first appointment. Click here to get Adobe Reader.

What to expect

On your first visit, your therapist will perform a full postural assessment to determine possible asymmetries in your structure that may be contributing to your pain or dysfunction. You may want to bring shorts and a sports bra for this assessment. Appointments generally last an hour, however you may want to allow a little longer for the your first visit in case there are any questions.

 

Cancellation Policy

We understand that schedules do change, but we ask that you give us 24 hours notice if you need to cancel an appointment, otherwise a $50 cancellation fee will be charged to your account.
